Sutter Health Workshop: "Aging Well: Navigating Post-Acute Care"
This workshop will provide valuable information for older adults about what happens after a hospital stay—specifically the types of care available before returning home. From skilled nursing facilities and acute rehab centers to short-term and long-term care options, you’ll gain a better understanding of how to make informed decisions, plan ahead, and advocate for the best possible outcomes.
